Rethinking the Personal Health Record

Too often health information is presented in an unwieldy and unintelligible way that makes it hard for patients, their caregivers, and their physicians to use.

Current health records are essentially plain-text ASCII dumps of information from a database that are difficult to interpret or understand either as an individual or a medical professional.

The following is my submission to the Health Design Challenge which I was lucky enough to win for the category Best Problem/Medical History.
